Carer Wellness at Home
We recognise that carers can often struggle to find time to access support for themselves. With this in mind we now provide an in-home program that will bring our service and support to you.
The Carer Wellness at Home Program is intended to support carers who may have difficulty accessing office-based services and who require assistance with:
- service navigation
- aged care information
- aged care reform information
- referral pathways
- practical assistance
- well-being support
- community connection and/or
- access to available support and services
Eligibility
For carers to access the Carer Wellness at Home Program, the person they care for must be:
- 65 years or over (for Aboriginal and Torres Strait Islanders 50 years or over)
- Has been assessed as eligible for Australian Government funded aged care services through My Aged Care
- Is receiving Commonwealth Home Support Program (CHSP) services, Support at Home services, or is awaiting commencement of approved aged care support
- Resides within the program’s designated service area
Carers may self-refer or be referred by another service, organisation, professional, family member or Carers WA program.
