As a service provider, you play an important role in identifying and supporting carers in your community. When you recognise someone in a caring role, a simple conversation could connect them with valuable support to help them continue caring with confidence.
Carers Wellness at Home is a free service that offers personalised support for carers of older people. Carers can meet with a Carer Wellness Support Officer either in their home or at a community location, where they can discuss their caring responsibilities and learn about the services and supports available to them.
Carers Wellness at Home provides:
- Individualised and ongoing carer support
- Assistance with completing paperwork and forms
- Guidance navigating My Aged Care and Carer Gateway
- Referrals to emotional and practical support services
- Access to wellness and educational events
Eligibility
- Over 65 years of age (50 years or over for Aboriginal and Torres Strait Islanders)
- Receiving Commonwealth Home Support Program (CHSP), Support at Home services or is awaiting commencement of approved aged care support.
- Resides within the program’s designated service area, we travel within the metro area; North as far as Two Rocks, East as far as Chidlow, and South as far as Port Kennedy.
Referrals
The Single Assessment System, other services and carers can refer into the Carer Wellness at Home Program by completing the specific Carer Wellness at Home Referral Form by downloading the referral form available here, which can then be submitted to wellnessprogram@carerswa.asn.au.
