Rural and Remote Health Outcomes Monitoring Platform Project (RHOMP)
The project aims to better understand and improve care for people living in rural and remote communities across WA. By linking data from hospitals, primary care, retrieval, and community services, RHOMP will provide a system-wide view of care pathways and identify opportunities to make care safer, more coordinated, and more responsive to community needs. Our initial focus areas include kidney disease, diabetes, and cardiovascular disease, with future expansion into maternity care and mental health.
We’re currently inviting consumer and carer input to help guide and shape this work. As part of this, we’re establishing a Consumer Advisory Group (CAG) to ensure the project reflects what matters most to people and their carers in rural and remote communities.
Session involving approximately three hours total (including preparation and follow-up). Engagement is expected up to four times per year and does not require a long-term commitment.
