South Metropolitan Health Service
South Metropolitan Health Service (SMHS) is seeking health consumers to join a new Community Services Consumer Advisory Group (CAG). SMHS Community Services provide hospital in the home services, along with metropolitan wide allied health, rehabilitation and care coordination services to patients who have been referred to our service from a hospital or via your GP. The purpose of this group will be to provide input into the way our services are delivered to make sure we provide the best care for our consumers and carers.
The membership of the committee aims to include patients and carers, along with Community Services staff. The functions of this exciting new committee will mostly include the review of:
71-73 Norma Road Myaree or online via MS TEAMS
2 hours quarterly – ongoing (minimum 1 year commitment)
