Future Living warmly invite carers to a free Advance Care Planning Workshop as part of National Carers Week 2025.

This workshop will offer valuable insights and practical guidance on:

• What’s involved in Advance Care Planning.
• Exploring what matters most to you and your priorities for the last stage of life.
• How to have meaningful conversations about your wishes with family, loved ones, and healthcare providers.
• Understanding palliative care—what it is and when it should be offered.
• How to complete important documents such as the Advance Health Directive and Enduring Power of Guardianship.

While the workshop is designed to inform and empower carers, we will also be celebrating carers and all that you do.

The workshop will be hosted by Palliative Care WA.  Supported by Carers WA and Lotterywest.
